I managed to wrap up my goal to enter all Ascendant Challenges from out of map by finally adding Keep of Honed Edges to the list. The trick was to get their via the Confluence, which as u/JewBoy300 reminded me, does not have a box once you load the adjoining regions. You also have to follow Toland back to the exit and take the path his little orb takes (right vs left). You can drop the sword and pick it back up but do need to take it all the way to the exit to finish the activity.
